Incident Reports
An incident report must be completed within 24 hours any time an incident occurs that involves actual or potential
- Property damage
- Occupational injury/illness
- Harm to patients, associates, medical staff, volunteers or visitors.
When in doubt fill it out. Be sure to include falls, blood, exposure, equipment malfunctions and theft.
Steps for Reporting an Incident
- Report an injury to your immediate supervisor and Volunteer Services immediately; the supervisor will complete the necessary incident report
- If an injury appears to need medical attention, report to Employee Health immediately or contact the house supervisor on evenings and weekends
- Incident reports are confidential, do not discuss them with associates or volunteers
- Should you need medical treatment, please receive authorization from Employee Health prior to being treated by a physician.
