It is perfectly normal to have questions or concerns during your hospitalization. St. Joseph Medical Center assigns a dedicated case manager to each patient floor to assist patients and their families.We charge no fee for this service.Your case manager can answer questions and address concerns, both during your hospitalization and discharge.
Your case manager will work closely with your physician and the hospital staff to ensure that you receive the best care possible.Case managers can help with:
- Facilitating discharge planning with your physician and coordinating home health care or, if needed, your transfer to a skilled, intermediate or residential care facility
- Offering support and assisting in coordinating services for patients in our rehabilitation units who require physical, speech and occupational therapy
- Assisting new mothers with nutritional information, counseling and addressing financial concerns; they can also offer assistance to birth parents who have made an adoption plan for their child and provide special support for parents who are coping with the illness or loss of a newborn
- Trained social workers on staff can provide counseling and offer emotional support and assistance to you and your family
- Working with community agencies to provide support and education to patients and their families; they can also help patients prepare advance directives, living wills or durable powers of attorney.
